USPS Form 1583 is the Application to Receive Mail through a 3rd party.  You as the Applicant designate Virtual Offices of Las Vegas as your Agent to receive mail on your behalf.  USPS requires this form be notarized when not completed in the Agent’s presence.. Two forms of identification is required to provide, to verify you and verify your home address.  This document serves as an official acknowledgment of being your mail-receiving agent. It gives the USPS the data to validate and verify your mail reception arrangements.
